My overall experience with Align Surgical Associates has been positive. Appointments were made on a timely basis and the staff answered my questions. The doctor was knowledgable and helped me choose the correct procedures for me.
I had an initial consultation with Dr. Safir at Align for Orchiectomy and Vaginoplasty, and while the time to schedule a consult was super quick (only ~1month wait compared to the nearly 5 months I waited for my FFS consult with Dr. Mittermiller), it seems like the time to actually schedule any procedure is absurd. While I am excited to move forward with FFS and Orchi, I am still not 100 percent sure I am going to go with Dr. Safir for Vaginoplasty, as I am hesitant to 'settle' for PIV, especially when there are so many newer procedures that offer better aesthetics, self lubrication, and potential depth. Further, the surgery scheduling team feels downright incompetent at worst, or just severely understaffed at best, why even email me about scheduling if you are not going to even try to schedule my procedures for more than a month.
Dr. Mittermiller does not have a warm bedside manner nor the ability to define a clear surgical plan. We discussed all of the possible facial procedures and we agreed on one, then decided to get a CT to check for other bone contouring. At the end of the visit he asked if he could text my previous facial plastic surgeon (that he already knew) that he saw me. I said 'yes' because I felt uncomfortable saying otherwise. Then shortly after my appointment he asked to speak with my 'mental health provider' via his nurse through email. I asked why he would need to speak to her and was told that Dr. Mittermiller wanted to confirm that my recent switch to a different psych medication meant that I was still stable. I signed a release only allowing him to speak with my psychiatrist regarding my medication change. After they spoke, I was told by my psychiatrist that he never mentioned my medication change and only wanted to discuss my desire for doing a revision surgery, something that was not discussed in our initial visit nor in the email. I was upset that he wanted to speak with her under false pretenses, but ultimately decided it was not worth making an issue of. I then received a second email requesting to speak with my therapist. I was told by his nurse that it was to see if my mental health would be significantly improved through doing this surgery, something already detailed in her letter. After they spoke, she had told me that he was very inquisitive about the procedures I had inquired about and essentially wanted her to assuage his worries. She did, and at this point I got the CT scan. When he called me to review it, he told me that there was nothing that he could do. He said that if he did any revisions that there would be significant damage to my scalp and a poor cosmetic outcome. At this point, I was frustrated having had jumped through so many hoops just for him to change his mind. He said although there is additional contouring that can be done, he is not comfortable doing a revision. I completely understand his declination, but I would've appreciated it before having to go through all of these additional steps. I still believe that he is a good doctor who does wonderful work, however, his inability to be direct and communicate with his patients and his inability to do revision procedures is worrisome.

Dr Mittermiller was great - very thorough, willing to talk through all my questions and get in depth about procedures, goals, recovery and so on. The clinic staff were friendly as well. The pre-appointment admin requirements (forms, photographs, etc) were much more onerous than I have experienced with other FFS consultations. It certainly wasn't enough to put me off, but I want to note it for completeness.
