Fairview Nursing Care Center offers outstanding short-term rehabilitation for individuals coming from an acute-care setting, as well as superior long-term care and more. Fairview puts a special emphasis on reducing the number of hospital re-admits through their unique Re-Hospitalization Prevention Program, which focuses on CHF/COPD.

The Rehabilitation Team at Fairview brings hope, perseverance, resourcefulness, creativity and dedication to our residents. We implement out-of-the-box thinking to help our residents overcome obstacles and achieve success. Our rehab department provides therapy for residents who have suffered a stroke, orthopedic injury or surgery for joint replacement, traumatic brain injury or other neurological conditions. Physical, occupational and speech therapy are offered. We enable residents to take back control and aim for independence. Together we find ways to adapt function to their abilities and to help them achieve more than they thought they would.
CHF/COPD Program
Our Proactive Disease Management Education program teaches our residents about disease processes, symptom management and behavior modifications: ie. a low sodium diet, exercise and weight management. CHF Weight Guidelines are carefully reviewed and documented with the patient and their caregivers. Additionally, Telemonitors are incorporated into our Plan of Care for High Risk Patients.
Long Term Care
Here at Fairview Nursing Care Center we offer a full range of services aimed in helping those residents with Alzheimer's and Dementia. Our program aims to let residents enjoy a sense of independence while still being under the constant watch and care of our skilled staff. Based on our residents' specific needs we design an individualized care plan for each resident. We try to identify things that are unique to the resident and help them decorate their rooms with things from home to make Fairview Nursing Care Center feel more familiar. Our comprehensive care programs maximizes results and gives each resident what he or she needs in terms of care.
