This provider is publicizing MY longtime home address as his place of business. His posted Kansas City address is a private residence, not a surgery center! The Missouri phone number he posts online is a non-working number. He lists a contact number in Ohio which is a non-working number and shows he's licensed in Ohio but lives in another state. Be careful!
