The only complaint I have is they removed my mother's 14k medic alert bracelet and lost it. It has been a month and a half and they still have done nothing about it.

Last year, my mom stayed there for about 6 months. She had special dietary needs: fresh fruits and vegetables

They were not able to supply this - cited high costs. I don't think bananas is a

high cost item. They ran out several times. Also, the staff was not as friendly as I've seen in the place where my

mom is now. I would highly recommend another place.

My mom was admitted to Orchard Park on 11/19/09 after a week long stay in the hospital because she had fallen twice at home and needed physical and occupational therapy to regain her strength.

The facility was verbally informed of her high fall risk, had received all discharge papers from the hospital, and had themselves identified her as a "high fall risk".

The following incident occurred on 11/20/09.
8:20am - I arrived at Orchard Park and my mom complained of feeling a little dizzy, foggy and mentally unclear. She had vomited the night before. She thought her medication doses might not be correct.

9:00am - We spoke with Sarah (head nurse) about the medication, let her know my mom was not feeling well, and that her medication, Tegratol, for neuralgia might need to be adjusted.

9:30am - An assistant came in to ask if my mom wanted to shower. She said no. The assistant said she would need to take her out of the room to weigh her and proceeded to put a wheelchair by the bed and wait for my mom to get into the wheelchair. I said to the assistant, "She is not strong enough to get out of bed, you have to help her." I was surprised and concerned that I needed to give her this information as we were very clear with Melissa McPherson and the nurses that my mom was weak, just out of the hospital, barely able to feed herself, and was at a high fall risk. We asked about getting a bed alarm and side assist rails (neither of which were done.) At my request the assistant helped my mom into the chair.

10am - I turned on the TV and left the room to exercise telling my mom I would be back in an hour.

10:45am - Someone came into the room where I was and said my mom had fallen. I ran into the room and there were about 4 people around her, there was blood on her hands, head and around her on the floor. One of the nurses had a bandage pressed to mom's head because she had been on Cumadin and her blood was thin. An ambulance had been called. I asked what happened and someone said she was sitting on the side of the bed and was trying to get up when she fell. Then, Kenny Thomas, the physical therapist on duty told me that he was actually working with my mom. What he told me at the time was that she was sitting on the side of the bed and he asked her to stand and she kind of "blacked out" and fell. He said he tried to catch her but he couldn't and she hit the ground. I asked, "why weren't you holding on to my mom?" He responded, "I've been doing this for 13 years. I tried to catch her, you have to trust me."

She was not wearing an assist belt at the time and no one had a hold of her. Whether or not she "blacked out" is beside the point. The nursing home acted with negligence that resulted in immediate and grave harm to my mother. It is inexcusable that a woman in my mom's condition was asked to get up by herself without the assistant or physical therapist using an assist belt at all times. The result of her fall (it looked like her temple hit the corner of the window sill) was that she was taken to the emergency room and she had 4 deep tissue stitches and 8 superficial stitches. The wound was a full laceration thickness through all of her tissues above her right eye. She also had a small skin flap wound on her right hand and a cut on her left thumb. Her right eyelid was swollen and bruised. She stayed in the hospital for another 3 days to recover until we moved her to another nursing home facility.

5:20pm - I and my sisters went to Orchard Park to meet with the administration and to retrieve my mom's personal belongings. None of the administrators showed concern, remorse or empathy for the situation. They asked in a very curt tone of voice "what questions we had." The "report" they gave us at that time was that Mr. Thomas was at the end of my mom’s bed trying to help her get her feet to the ground when she fell which is why he could not reach her in time. (not the story Kenny gave at the time). We are very concerned and dissatisfied with the "care" my mom received at Orchard Park Health and Rehabilitation Center and the subsequent lack of care and consideration shown by the administration. Please make this complaint public record. We are concerned about the care of other prospective patients and would advise others to look elsewhere for a more caring, trained staff of professionals.
