I want to thank the people and staff of Heritage Park for the courtesy shown to me and my family during my stay there. While there I was treated great and was very well taken care of. I also was very impressed with the care they gave to the people and families of the people who were passing away. They were treated with respect and care, and well taken care of.

I was very impressed with the care and attention they gave to everyone under their care, even those who were not treating them with the thanks that they deserved.

I had a loved one at heritagepark and was truely impressed with the stay. The staff were very nice and helpful, and the therapy was great. All the nurses really helped make my loved one comfortable and get the best care they deserved. It is always difficult to have a loved one in a nursing home, but the staff at heritage made her feel at home while she recovered.

This place is horrible i wouldnt put an animal in here none the less a loved one! Heritage Park is so nasty the nursing staff and CNAs a awesome but the management is horrible do not take your family member here please its so bad the food is so nasty the bathrooms are gross i cant believe they make their residents use them i wouldnt go ten feet near them. just never even consider this place it seriously needs to be shut down!

I have checked out the local nursing homes in the area(Ogden).I have to say at first I was worried BUT- BIG CHANGES in the nursing area. There isa new director of nursing and she has made many changes. She has been great with my concerns...answered my questions and has had everyone on board!
I love the lay out of the building, my Dad doesnt feel cramped in a room. Physical therapy people have been AWESOME!!!
I am glad we took Dad there, he likes it - everyone is very kinda and he loves all the activity!!!
I feel he is SAFE and cared for everyday!!!!

Does this provider promptly return your phone calls?
Absolutely not! My phone calls aren't returned at all!
Were the staff members able to answer your questions?
Every time I asked the staff a question, they referred me to someone else
Were you able to get answers to simple questions from this provider without scheduling an appointment?
Not at all! I had to schedule an appointment for absolutely everything!
How would you compare this provider to others in his/her field that you have visited?
The worst I have seen
Does this provider remember you and your circumstances at every appointment?
Absolutely not! I constantly have to repeat and re-explain my situation!

If you have a loved one....DO NOT TAKE THEM TO HERITAGE PARK CARE CENTER. My sister-in-law was there 3 months for rehab. Horrible experience. It was all medicare would pay for. They do not clean their patients, continually lose their clothes, let them sit in the poop, and the food is terrible. Not sure how they stay in business. The nurses and the CNA's are very nice and try to care for the patients best they can (overworked and unpaid) but all of the Management sucks! They are just worried about $$$$. The Director of Nursing, Stephanie is a big snob. I feel sorry for her husband and the nursings/CNA's working under her. They continually have a turn over of nurses because they treat their nurses/CNA's like crap.

High on cleanliness and appearance in the public areas and short term care but has more odors and smell as you go to the other areas. Older building, the rooms are smaller. A homey feel and great activities however have seen residents in chairs slumpped without support, staft texting and on phone sitting on chairs by the door so they could observe all but tells me they are not using that time to engage with residents - so a negative on quality of care. Issues regarding resident care on same issues have been repetative for the last year. Food/kitchen doesn't appear well managed. Portions to some are stingy. Kitchen staff seemed nice.. It wasn't the worst I've ever smelled, but it definitely wasn't the best. The restrooms were very well-stocked and never smelled however didnt have seat protectors. There was lots of available parking at their office. Bus service doesn't run on sunday so visiting family or church on sunday is non-existant. Not having stayed at other facilites, I'm not sure if it would end up being the same way all over but hope this can't be true. Comparing them to others in their field, I would say they would be on the low end due to delays in response or action.

Definitely, it was nauseating

Absolutely Not! He/She always looks like a slob.
