The folks at Greenleaf are so friendly. The staff do an excellent job of providing immediate attention to those who need assistance. My mother recently stayed at Greenleaf Nursing and Rehab Center for IV medication and rehab services, for a total of almost a month ½. It is the best rehab facility we've experienced. When I would visit my mother there was so much activity all around with patients in therapy and/or family visiting them. This would put my mind at ease. All the food was not the best, sadly. However, some meals were tasty, but it's the typical “nursing home” type of food. The rooms in the facility seemed to be all semi-private & my mom’s roommate was very nice. The administrator couldn't be nicer. I could tell right away he’s sincerely devoted to his work there. And, let's not forget about cleanliness. I remarked to NHA that I have never seen such clean floors in a senior care facility. I could practically see my reflection. They were polished like a jewel, truly amazing. I love visiting my mom and seeing the smiles on the residents faces. My favorite time to visit is Fridays, because they have a live singer and watching the residents clap their hands and dance, is the highlight of my day!! Hats off to the NHA and wonderful staff.

My mother was admitted to Greenleaf Nursing and Rehabilitation/Skilled Nursing Facility after suffering a stroke this past February. She was unable to walk, talk, or even feed herself but 3 weeks later she was not only talking and walking but was our old mom again. She was very depressed and didn't hold out much hope for recovery but the care she received here was comprehensive and the nurses, therapist, and staff was top notch. The room we a semi-private, which was great for mom, because she had someone to keep her company. I wholeheartedly recommend Greenleaf, they exceeded our expectations.

I recently had surgery on my leg and need extensive wound care. I spent about 4 weeks at Greenleaf Nursing and Rehabilitation Center. Overall, I had an extremely positive experience. The nurses, aides, and physical therapists were all well-trained, professional, caring, and went out of their way to be attentive and accommodating. Everyone I met was extraordinarily nice. The facility is an older building, but don't let the interior fool you, because their care was A+!! Overall a great facility that I would highly recommend to anyone. If you don’t care about an older building and would rather have great care, go to Greenleaf!!

We've made a decision and decided to move my father here at Greenleaf Nursing Rehabilitation Center,  because they offered a wide range of medical services, activities, and long -term care options.  The staff has been extremely caring and they really take their time to get to know you.  However, Its an older building, but we didn't mind, because at the end of the day, its the care that matters!! Our father has been extremely happy and has made a lot of close friends, in a short amount of time.  The building is well maintained, and close to where we currently live. I am very happy overall and I am happy we choose Greenleaf!!

My mother was recently sent to Greenleaf for severe COPD and multiple medical issues. From the PT's, OT's, nurses, docs and social workers, she could not have received better care, professionalism, attentiveness, compassion and enthusiasm. She developed a great bond with her caregivers which gave me peace of mind.
I have no agenda to support this place for my own or someone else's advantage, just to offer appreciation for a job well done. I recognize this is challenging work and good medical staff does not get enough appreciation for their tireless efforts. Thanks to everyone in Greenleaf, I appreciate you all for your patience, your skills and your positive attitude with my loved one.

Our mother moved to Greenleaf over 1 year ago and our family has found the facility, staff, and grounds to be everything advertised. The staff is attentive and caring, and we appreciate the fact that they regularly kept us advised of any changes, events, etc. whether they were critical in nature or not. The caring nature of the staff, from her nurses and aides to the administrative personnel and the people who coordinate and lead the various activities. This is so important to us, as we visit often but do not live close by.

The worst place you could possibly even think about placing a human being in. Dreadful care. Telling you to only call if you pooped yourself and not if you have peed in your diaper. You end up with dreadful sores that when admitted to a proper hospital, the nurses are appalled at the condition you have been enduring. When a patient cannot walk, there should be constant hygiene given to them. Especially when the clients are paying for their care. Specifically one caregiver stating that your are lucky you are alive, because she would have killed you. For heavens sakes, do not place anyone in this hell hole. If you fall out of bed, they throw you back in. Throw you!. They play music loud late at night. They do not quieten down the roommate you are forced to be in the same room with. They leave you outside during the night and forget you are there. I was left outside in the freezing cold night, unable to walk. They are horrendous. April only wants your money, she does not care what goes on. Mentally insane people live there, and some even attack you. .The caregivers are underpaid and constantly complain about working there, and readily state that they cannot wait to find a better place to work. They are grumpy when they serve you meals in the dining room. I don't think they realise that not everyone is out of their minds, some just need nursing care. Don't spend a dime or a thought on going to this hell hole

My father in law was recently a resident here. It was the worse place I have ever been. My husband and I live out of state so when the hospital placed him they recommended this place. It is very dark and dim, the hallways are narrow, and has bad odor. Residents stand by the door and try to get out. There is no security. We were not greeted in a friendly manner, and the nursing staff are incompetent. My father in law waited over 40 minutes for his call bell to be answered this is while we visited. My husband in the field assisted him to the bathroom but we waited to see if staff would come. When they did they stated "why did you ring the call bell if you didnt need us" and walked out of the room. We asked to speak with supervisor or someone in charge and they stated "we are short no one is in charge," We left several messages for the administrator but no one returned our phone call. We even reached out to the social worker but no return call. Eventually my father in law went back to the hosptial.

My MIL has been in this establishment for EIGHT years and after countless 'care conferences' to improve her quality of life the staff and this organization have repeatedly let us down. I would NOT wish anyone be sent to this establishment. Anything not nailed down could be stolen INCLUDING CHRISTMAS GIFTS!!! and any requests fall on deaf ears. The staff turns over SO MUCH that no one ever seems accountable. This used to be a fun place. They had resident parades, fun activities to get people outside into the sun but now so many residents have tried to escape that NO ONE is allowed outside without family or a pass from the staff to go smoke (My MIL doesn't smoke) I hate feeling like my hands are tied when it comes to her care, I am actively looking to get her out of there. ANY help would be appreciated. Thank you for reading.

I didn't find this Nursing Home to be as caring as I would have liked for my mother. She was in so much pain all the time. When you have a patient who is clearly in such distress someone needs to take it upon themselves to get proper help for them. Giving narcotics is not a way to treat someone without a diagnosis. Your staff just added to the problem and never offered any help. Your outlook was just load them up so the screaming stops !!
That is no way to take care of patients. Not one person ever came in to check and see if anything was ever needed. I would never send anyone here for care and it should be a non-smoking facility !! The halls stink like smoke !
