Have been a patient of Dr. Jak for 12 years. Great care and super staff. Love all of them. They have been very good to me.

Wonderful doctor! He took care of me in the hospital and then later for sleep study for the last several years. Dr. Jak, was always very thorough and has a great, friendly personality. His office staff is wonderful as well. I have since moved out of state and hope to find a doctor as great.
