I gave this provider one star for the following reasons
1) Not progressing the patients treatment plan while having a chronic condition
2) Seems confused and forgetful
3) Does not seems to care about patients well being
4) Poor hygiene
5) failure to follow up with patient care

I would avoid this provider if possible
