dear dr.david arnold .my name is stoney c. leachman.i saw you back in 1999 for my back problems .you gave me a total disability paper, i need you to send me another one.the s.s.a. didnt go by it.i dont know if they got it.my primary dr. is dr .robert marshburn in commerce,ga.you can send a copy to him if you prefer,it would be greatly appeciated.
