Never, they are very reliable.

The office does not call back for days on end when trying to call in refills for prescriptions. I don't know if this is the doctor's doing or a poor office staff - but it's frustrating - personally I have been waiting 9 days so far...no return phone calls from the doctors office and no calls to the pharmacy.
