The reason I at this have to continue to see Jazayeri is due to his staff employee,Janice Lawson to whom have since the beginning of myself becoming a patient has repeatedly neglected to properly do her job, regarding my medical needs and request per dr.s orders. She would put the blame on him stating he wasn't putting in the orders. By the time things were ordered my life became very hectic. I wanted to take care of my medical needs a little at a time due too anxieties and Janice easily caused unnecessary stressed. As long as she did her part, my part was taken care of. My medical records to this day are severely damaging to myself on regards to to the treatment I require as a permenantly disabled female. She's been made aware of this and instead ignored any of my requests for corrections and amendments since 2014. I've recently been made aware of more misrepresentation regarding my medical file. So I will let the doctor know that this information needs to be corrected so I can find a suitable doctor without having to explain myself.
