My mother asked Dr. Mantha for a DMV disabled placard. Dr. Mantha claimed "DMV doesn't give us the form". I find this excuse just amazing. DMV has the form online and it's only a couple of clicks into their website to get it. Obviously Dr. Mantha's staff just could be bothered with such things. I'm disgusted.
