We were charged for phenomena shots which we did not get as I have had several and AMA suggests one is for a lifetime. I was also charged an administration fee for this non injection experience. My partner was also charged for the same and did not receive and the office visit level was excessive. We are a PPO patients and should not receive any bills that were not covered. It is the responsibility of the provider to inform the patient that the procedure may not be covered and quote a price for the procedure.
