Michael Kozak
Dr. Kozak received his undergraduate degree from the University of Notre Dame, master's degree in public affairs from Indiana University, and medical degree from the University of Alabama School of Medicine in Birmingham. He completed his residency at Memorial Family Medicine Residency Program in South Bend. He is board-certified in family medicine and currently practices at HealthLinc Centennial.In addition to providing broad spectrum care for adults and children, he also has special interest in the management of chronic disease and HIVcare.
