Continuum is committed to meeting care needs at every stage of life.  With flexible availability from 2 to 24 hours a day, we are here when you need us.  Continuum’s social workers, gerontologists and nurses provide a FREE home consultation to discuss our services and your needs.  Subsidized services are available for those unable to pay privately. 

We differentiate ourselves by incorporating a care management model, which uses professional social workers and nursing staff to manage each client’s care.  Regular supervisory visits to every client and client/caregiver introductions give assurance that the right care for each individual (anything from personal care to meal preparation) is being provided.  You will never have to worry about a lapse in coverage, due to your caregiver being sick or car troubles because we guarantee service.
