About
EldersChoice is unique. We provide a free assessment in the home, hospital, nursing home or rehabilitation center. If we are able to meet the client’s needs, we create a comprehensive individualized care plan that is followed by the caregiver. We screen and then place one highly qualified caregiver to live-in for one to three months before taking a break. We also offer live-in, respite care starting at a minimum of two weeks for individuals recovering from surgery who need at-home assistance and supervision on a short-term basis. We generally have caregivers available to start within 48 hours.
Our case management services assure that the care plan can adjust as the needs of the client change. We monitor each case, provide continuity with a temporary caregiver when the primary caregiver takes a break or becomes ill, and coordinate at home medical services, such as physical or speech therapy.
Best of all, EldersChoice costs are at least half the costs of hourly rates for 24 hour care and some long term care insurance policies will defray the costs of home care.
