Disorganized, poor internal communication resulting in poor or uneven responses, one nurse would come out and say one thing but that report would not get to the other nurses who would be unaware of the patient's deteriorating condition. Social worker, who is supposed to coordinate care, was clueless, did not keep family informed, and an airhead. We only kept them on because mother liked her primary nurse.
