State Center Specialty Care is a 46-bed skilled nursing facility offering rehabilitation services, skilled nursing care, long term care and hospice care. We are a part of Care Initiatives, Iowa’s largest not-for-profit skilled nursing and long term care provider. State Center Specialty Care accepts Medicare and Medicaid, and also serves as a contracted provider for the Veterans Affairs Community Nursing Home Program.

State Center Specialty Care has been recognized with the 2016 Bronze Commitment to Quality Award by the American Health Care Association.
