Brookhaven Health Care Facility

Welcome to Brookhaven Health Care Facility located in East Patchogue, New York. We’re proudly owned and operated by the McGuire family. For generations, the McGuire family and our employees have been committed to pioneering and integrating progressive programs and services that have defined our local health care community.

This commitment has resulted in Brookhaven continuously receiving outstanding ratings from the federal government, which rates every nursing facility in the country. To see our ranking, as well as those of other facilities, visit www.medicare.gov.

Brookhaven Health Care Facility is also proud to have a unique affiliation with Brookhaven Memorial Hospital Medical Center for outstanding rehabilitative care.

If you’re considering an elective joint replacement surgery, it’s wise to give yourself plenty of time to research all the rehabilitative options. This includes talking to your physician, visiting rehabilitation facilities, speaking with therapists and beginning the pre-planning process.

It’s always helpful to keep a few key elements in mind while conducting your research. While touring the rehabilitation centers, take note of the environment. Brookhaven Health Care Facility features specialized equipment as well as areas designed specifically for training in kitchen, bedroom, bathroom and laundry settings. This will help you master the skills necessary to return back to the community safely and successfully.

Statistics show, the most effective method for recovery is through a team approach. You’ll want to choose a facility that welcomes your collaboration on a rehab plan that meets your specific needs. For example, if you want to swing a golf club as soon as possible after a joint replacement, it’s important to have a skilled professional trained in orthopedics and sports rehabilitation involved in your recovery.

At Brookhaven Health Care Facility, your care is fostered by experts in every aspect of healing. Rehabilitative therapists help you regain the physical stamina and range of motion needed to recover; physicians and nurses address the disease process and teach you how to manage your medications; and dietitians assist in helping you adopt healthy lifestyle changes and dietary habits if necessary. After your recovery, social workers help you transition back into the community and coordinate home-based services.

Outpatient therapy is also an important feature to consider. It’s wise to choose a facility that offers both inpatient and outpatient services. This allows you to continue your treatment upon discharge with the same team of therapists and pick up right where you left off as an inpatient.

When touring the facility and talking to the therapists, inquire as to what a typical day might be like. It’s important to exercise after surgery to increase strength and flexibility in your new joint and to prevent blood clots. You may feel uncomfortable at first, but exercising will help you to get back to your normal everyday activities sooner.

If you’ve recently had a hip or knee replacement you will have a number of important regimens in your therapy schedule, some of which may include neuromuscular training, isometric strengthening of the thigh and buttock muscles and exercises to strengthen the hip muscles. Stationary biking or upper body cycling may be added for endurance as well. Progression of standing exercises, including walking backwards or on uneven surfaces, play an important role in improving balance as do a number of step exercises including step ups and side stepping.

If you’re deconditioned due to an accident or illness, your treatment may include a combination of physical, occupational and speech therapy. Rehabilitation will help you to get moving again, regain your strength, relearn skills or find new ways of completing tasks.

A final factor to consider is the facility’s overall ratings both at the state and federal level. Brookhaven continuously receives outstanding ratings from the federal government, which assesses every facility in the country. More information can be found at www.medicare.gov. The ratings are important because it’s a reflection of the overall culture of quality that exists at every level within a facility. It’s the highest quality facilities that generate the best outcomes for patients with intensive care, faster healing and less chance for problems or rehospitalization in the future.

Services Over the years, Brookhaven Health Care Facility's programs have grown in scope and complexity from 24-hour skilled nursing care to subacute & outpatient rehabilitation, palliative care, and respite/short-term care for individuals seeking temporary options for a loved one.

Brookhaven Health Care Facility is also the first facility on Long Island to open a TeliStat™ Restorative Care Unit (TRCU). The TRCU concept and the associated proprietary technology was developed by Dr. Anthony Bacchi, and is designed to treat higher acuity patients more effectively in a nursing home setting.

The TRCU delivery model provides patients with noninvasive monitoring of their cardiac performance through state-of-the-art telemetry. This includes the ability to monitor essential vital signs in real-time, thereby enabling physicians to intervene immediately in serious medical situations. Every patient admitted to the unit is outfitted with a portable wi-fi/cellular cardiac monitor, which registers the real-time data 24 hours a day, including when they are at therapy. The information is transmitted to a central nursing station, where our team has immediate access to any change in a patient’s status or condition.

The unit’s state-of-the-art capabilities help limit hospital readmissions by providing immediate medical intervention and decreasing the chances for adverse outcomes.

Professionally trained nurses are an integral component of the TRCU and have a 6-to-1 ratio with their patients. They are certified in advanced cardiac life support, intravenous IV insertion and have telemetry expertise. Patients are cared for by an interdisciplinary team, including doctors, registered nurses and certified nursing assistants. Clinical oversight by a cardiologist consultant and intensivist is available 24/7.

The only plus is the staff has been nice to my grandmother... The food on the other hand is awful. Its a load of processed junk. Every time I would go visit my grandmother I would see peanut butter and jelly sandwiches on white bread and macaroni with cheese, or spaghetti. How are these patients getting any nutrition? My grandmother always had a healthy weight until she came to this place. She can't even walk anymore.

The nurses gather patients, like cattle, in a small room, right off the nurses station and do not allow them to go back to their room or to use the bathroon. I have known 3 2people that have used this facility. They both hadf the same complaint, as above. The people at the nurses station, on the infamous "Unit A," on the evening shift are all part timers, who work at hospitals, so, and I quote the Nursing Supervisor I spoke with, "They're all part timers and they really don't care as much as we'de like them to." I was told that, after being repeatedly hung up on, for calling at that specified time, by the nurse supervisor, asking to speak to my father. Now, the food is excellent. The place is spotless. The physical therapy team is unrivaled. But, the evening nursing care makes this facility a HUGE 2 thumbs down. Avaid this place. You'd be MUCH better off taking the extra few miles and using a place that is in West Hampton.
